General Steps
To enroll an eligible patient in one of our programs, please complete the following steps
according to your region: US, Japan, Europe, and Other Countries.

Labcorp Account Setup
Health care providers must obtain a Labcorp account specific for the RDSTP (including providers previously registered with Labcorp). Account setup takes 10 to 15 business days in countries outside the United States and 3 to 5 business days within the United States.
To setup an account, health care providers must complete a Labcorp’s online account setup form or a PDF account setup form. *PDF form must be sent to rarediseaseprogram@labcorp.com

In-Servicing
Upon completion of account setup, Labcorp will provide the main account contact with detailed information about the program, including Test Requisition Forms (TRFs), sample collection guidelines, online portal instructions, and patient service centers information (US only).
For health care providers in Japan, Europe, and other countries, a Marken starter pack will be delivered in addition to Labcorp’s information.
All these In-Servicing documents will be sent electronically.

Test Ordering
Available options include:
1. Utilization of one of Labcorp’s patient service centers (PSCs) for phlebotomy and sample transportation.
• Visit www.Labcorp.com and click on “Make a Lab Appointment” to find the nearest PSC location and schedule an appointment.
• Patients must attend to their appointment with their completed TRF and a photo ID.
2. Sample collection by the health care provider in compliance with Labcorp’s collection requirements and subsequent pickup of samples by Labcorp (call at 1-855-LABCORP).
• Samples collection supplies are only available for US and Japan. Within the US, health care providers can call 855-LABCORP and follow the prompts for ordering supplies.
• Note that depending on the tests to be ordered, you must have to complete the appropriate TRF for each patient. • Mandatory information that must be included: patient name, date of birth, sex, provider name, and test requested. Treatment must be marked for immunogenicity testing.
• Health care providers must complete the appropriate Test Requisition Form (TRF) according to each patient.

• All health care providers can utilize their own supplies to collect samples, as long as Labcorp’s requirements are met.
• Samples collection supplies are only available for US and Japan. Within Japan, health care providers can order supplies by faxing Marken Japan at 0120-430-122 or by emailing a completed Sample Collection Material Order Form (from Marken’s starter pack) to marken.japan@marken.com

Sample Pickup
• Samples collected at any patient service center (PSC), are automatically delivered to the Labcorp laboratories within 48 hours.
• For samples collected outside of a patient service center (PSC), samples will be picked up within 48 hours of request, by calling at 1-855-LABCORP.
• Please ensure that sample tube(s) are labeled with two unique patient identifiers that can be matched to the completed TRF(s) along with the sample type (ie, plasma, serum, or urine). Unlabeled or improperly labeled tubes may delay testing and resulting.
For Japan, Europe, and other countries, Marken is the sample transportation and logistics service provider for this program.
• To request pickup, health care providers will call their local Marken office or fax/email a completed Booking Form to their local Marken office.
• With 24 – 48 hours notice, Marken will pick up samples at a requested date and time.
• Marken will provide all necessary packaging for sample transport.
• For Japan only: specially trained Marken drivers will package samples and TRFs.
• For Europe and other countries: facility will need to package samples and corresponding TRFs (1 patient per biohazard bag) per instructions in the Marken starter pack and provide a completed invoice and air waybill to the driver at pickup.
• Samples will be delivered to Labcorp in the US within 14 business days of pickup.
• For adverse event testing, expedited shipping (less than 7 days) is available but must be requested by health care providers when scheduling sample pickup. HCPs must check off “ADVERSE EVENT” on the Booking Form when coordinating sample pickup so the shipment can be routed directly to the United States (bypass regional hubs). If calling in for pickup, HCPs should let Marken representative know that adverse event samples are included.

Results Delivery
Results are delivered directly to the ordering HCPs or Reference Laboratory by fax or through Labcorp’s online portal.
